Student & Parent Intake Form

To provide the most personalised guidance possible, we ask all new families to complete a short intake form before their first consultation. This helps our advisors understand your child's background, current situation, and goals — so your session is focused, productive, and tailored from the very first minute.

What to Prepare

Before completing the form, please have the following ready where possible:

  • Recent school reports or transcripts – Last 1–2 years of academic results
  • Marksheets or grade summaries – Subject-by-subject performance data
  • Any previous assessment or counselling reports – Psychometric, career, or learning assessments
  • Certificates of achievement – Academic, extracurricular, or community awards

What the Form Covers

  • Student personal details (name, age, year group, school, curriculum)
  • Parent/guardian contact information
  • Current subjects and academic performance overview
  • Interests, hobbies, and extracurricular activities
  • Initial career or university thoughts (if any)
  • Any specific concerns or goals you would like to address
  • File upload: marksheets, reports, and certificates

What Happens After Submission?

Once we receive your completed intake form, a MyEdulytics advisor will review the information and reach out within 1–2 business days to confirm your consultation appointment and share any preparation notes.
